Directions in Aging Services in Westchester

We consult with families to find effective strategies and make the right decisions about care for their aging family members.

Our strength is creative problem solving…helping families find answers to tough questions and manage difficult situations. Often aging parents resist accepting help or making changes in their living situation.  With over two decades of experience guiding families on such issues, we’ll help you find effective strategies to turn “No” into “Let’s try.”

Our services include:

Pre-Need Home Visits
Families find it practical and reassuring to meet Miriam when all is well. A home visit, without the stress of an emergency, is the perfect opening. It also gives Miriam the opportunity to gain an understanding of current concerns and long term needs.

Comprehensive Geriatric Assessment
We’ll make a home visit to become acquainted with your family member and conduct a thorough assessment. We pay close attention to medical issues, potential safety problems, the person’s ability to dress, bathe, prepare meals and pay bills, the use of home technologies, and the degree of insight and judgment he/she displays in day-to-day decision making.

Coordination of Homecare Services
Directions in Aging can help you find the right kind of in-home care. We offer guidance on choosing the most appropriate caregiver, whether it be a companion, home health aide, or a higher level of care. We can make regular home visits and help caregivers manage the challenging behaviors often associated with Alzheimer’s disease or other forms of dementia.

Liaison to Community Resources
Many families are new to the world of eldercare, and don’t know where to turn. We are deeply familiar with community resources and connect families with the programs and services they need, from elder care attorneys to caregiver support groups. We also assist in locating state and federal entitlement and benefits programs.

In-Home Support and Supervision
We can accompany your family member to doctors’ appointments, arrange for a home repair, a delivery, or whatever is necessary to keep an older adult safely at home. Ongoing communication with family members about the status of their parent or relative is always available.

Assistance in Making the Transition to or from Home to Independent Living, Assisted Living, or a Nursing Home
Based on a comprehensive evaluation, Directions in Aging will help identify facilities that best meet your family member’s needs. Once a residence is selected, we can assist with the move and ensure that the staff has essential information about the new resident. We can also arrange regular visits with your family member for the purposes of socialization, monitoring and advocacy.

Guidance with a Geographic Move
The time may come when a parent should live closer to family. Via our portal of Aging Life Care specialists throughout the United States, we will customize that move. Starting with arrangements for transportation, we will oversee the connection to a care manager who is thoroughly familiar with residential, medical, and community resources in your parent’s new location.
